Before the Numbers, We Start With You
Reflection changes how you engage with your own health. Before any result is reviewed, you work through a structured set of questions — where your health has been, where it stands today, why you're here, and where you want to go. Putting that into writing isn't administrative. It's the first act of owning your health rather than having health care happen to you.
The intake closes with an Authorship Statement — a plain, first-person declaration of what better health would make possible in your life. Writing it down matters. People who articulate what they're working toward, in their own words, stay more committed to the work. That's the point of authorship: your health becomes something you're actively building, not passively receiving.
The series doesn't end at your evaluation. Follow-up questionnaires at 90 days, 6 months, and 12 months bring you back to the same essential questions — tracking how your thinking has shifted, keeping you oriented toward your goals, and giving the care team what they need to adapt your plan as your life and priorities evolve.

Initial Evaluation & Diagnostics
Before any data is reviewed, you complete the Health Authorship intake — a structured reflection on your history, priorities, and goals that the physician reads first. From there: an unhurried physical exam and the advanced diagnostics that fit your situation — DEXA, VO₂ max, coronary calcium, advanced bloodwork, and more.
Personalized Plan
Your physician walks you through every finding — in plain language, without rushing — and delivers the Executive Health Report: a scored, written clinical document covering cardiovascular health, body composition, movement and performance capacity, metabolic function, sleep, nutrition, and more — with a prioritized 90-day roadmap built specifically around your data.
90-Day Review & Beyond
At day 90, every patient returns for a physician review — tracking what changed and setting the next direction. Foundation and Optimize membership extend that support for those who want continued team involvement over the long term.

No new testing required. You bring what you have; your physician reviews it, applies clinical context, and delivers a written Physician Guidance Note covering findings and recommended next steps.
